Icon went missing on 'Application Launch Bar'

I added the LMMS app to the Application Launch Bar and the LMMS icon appeared on the bar and everything was working fine, then all of the sudden the LMMS icon went missing on the bar and it was replaced by some generic icon indicating a missing icon (red circle with line through it).

When I go into the 'Application Launch Bar' edit box, it shows the LMMS icon on the left column of added apps like everything's OK.  Removing and re-added it doesn't fix it.  Adding other apps shows their icons just fine.

Was going to reinstall LMMS to see if that would fix the icon problem, but Synaptic only gives options to remove LMMS and I'm a little nervous to since it says the LMMS is a Custom AV Linux Build and not sure if uninstalling it and reinstalling will bring back the custom AV version.

Weirdest thing, the icon for Rosegarden doesn't show up either when adding it to the App Launch Bar.

I'll check out the icon issue when I get a minute...
